Help us get this questionnaire right

We're building a questionnaire about everyday thinking skills: starting tasks, planning, attention, time, emotions, and what helps. It's for adults who live with ADHD, autism, depression, bipolar disorder, anxiety, PTSD, OCD, learning differences or a brain injury, and for people who don't. Before we share it widely, we want people with lived experience and clinicians to read the draft and tell us what to change.

Is it clear?

Any word or question that's confusing, or could be read two ways.

Is it safe?

Anything that could upset someone, or feels like a test or a diagnosis.

Is it complete?

An answer option that's missing, or something important we don't ask.
